The four stars represent the Indians, Red Sox, Tigers, and White Sox.

This version of the logo was worn only by the Red Sox, Indians, Tigers, and White Sox - the only 1901 teams still operating in their original cities in 2001. While the Yankees, Twins, Orioles, and A's organizations can trace their histories to that first season, all had relocated at least once; and thus didn't qualify for the "Charter Member" patch.
